Pagets Disease


 History: 70-year-old male with history of prostate cancer, status post Prostatectomy. PSA level 0.25 ng/ml.  

Axial Fused 68GA-PSMA showing increased uptake in the left iliac bone. 
Axial CT image shows trabecular and cortical thickening with mottled appearance involving the left iliac bone. No underlying destructive lytic or sclerotic lesion.

Teaching points:

  • PSA level < 10ng/ml nearly excludes bone metastases.
  • PSMA uptake is secondary to overexpression of PSMA in the endothelial cells of neovasculature, probably stimulated by secreted angiogenic factors during bone remodeling in Paget's disease.
